Michigan online casino complaint checklist (printable)
A one-page checklist for a withdrawal problem or other dispute with a Michigan-licensed online casino or sportsbook, following the Michigan Gaming Control Board (MGCB) process. Print it, or save it as a PDF from your browser.
Quick answer
To complain about a Michigan-licensed online casino, first send the operator a written complaint; under MGCB form MGCB-AD-2064 it must investigate and reply in writing within 10 calendar days and tell you about your right to go to the Board. If it is still unresolved, send MGCB’s complaint form with the reply and your records.

Before you complain: is the site licensed in Michigan?
- The site or app appears on MGCB’s authorized providers list or Legal vs. Illegal Gaming page. The MGCB complaint process covers licensed operators and platform providers only.
- If it is not licensed, report it to MGCB instead: tip line 1-888-314-2682 or MIGamblingTip@michigan.gov (MGCB page).
What records to save, as it happens
- Screenshots of the withdrawal request and each status change, with date and time visible.
- Amount, payment method, and any transaction, reference or ticket numbers.
- Your account number and screen name or username (MGCB’s form asks for both).
- Full support chats and emails, exported or screenshotted.
- The part of the site’s terms you believe was not followed, as it read at the time.
- Date and time of the incident (the MGCB form asks for both).
Step 1: written complaint to the operator
- Find the operator’s complaint instructions. MGCB says complaint filing instructions are on all platform providers’ websites.
- Send the complaint in writing (email or a saved message), and keep a copy.
- Date sent: 10 calendar days later:
- Ticket or complaint number from the operator:
Per MGCB form MGCB-AD-2064, the operator or platform provider must investigate and give you a written response within 10 calendar days after receiving the complaint, and that response must tell you about your right to submit the complaint to the Board.

Step 2: if it is still unresolved, go to MGCB
- You have the operator’s written response. MGCB cannot consider the complaint until you filed in writing with the operator and received a written response.
- Fill in MGCB’s complaint form online through the Patron Dispute Form page, or use form MGCB-AD-2064.
- Attach the operator’s response and your supporting records.
- The form can be emailed to MGCB-IGaming-PatronDispute@michigan.gov or mailed to Michigan Gaming Control Board, 3062 West Grand Blvd., Suite L-700, Detroit, MI 48202-6062. MGCB phone: (313) 456-4100.
What happens after you file with MGCB?
According to form MGCB-AD-2064, you will receive notice that the Board received your complaint, and the Board may investigate and may direct the operator or platform provider to take corrective action. The form also says a complaint does not, on its own, open a disciplinary case, and that complainants are not told about investigative or disciplinary processes, which are confidential. A complaint does not guarantee a payout.
